This sounds very familiar to me, i was the same, while i manage to eat healthy during the day all good resolutions are gone once i settle down in front of the telly at nighttime.
This is what i do:
-not have any chocolate or sweets in the house,so u can't even get tempted.
-eat something healthy instead,like grapes, or have a hot chocolate drink,its not as bad as eating a bar of chocolate
-make sure u eat enough at the meals, you shouldn't feel hungry at nighttime any more
-if its just boredom that makes you want to snack,ring your friend or go for a walk instead
